Parents - Please PRINT, COMPLETE, and SUBMIT the registration packet below (once for EACH student) to register for youth camp. Also please complete the included "Alpine-Adventures-Waiver" form for each student and if the student is participating in the rappelling activity, please also complete and turn in that included waiver form as well.
You can scan and email the forms to office@greentownbaptist.com or turn in the printed copy to the church office.
